| The Provider Directory Search-Provider Directory panel is used by recipients to search for providers and community resources by different search criteria such as county, city, state, and zip code. |

| Field Descriptions: |
| clear | Clears all the search criteria. |
| search | Displays the Search Results based on the criteria entered on the search panel. |
| Business OR Last Name | Business or last name of the provider to search. |
| City | City of the provider to search. |
| County | County of the provider to search. |
| Program | Program type of the provider to search. Default is Medicaid. (This field to be determined.) |
| Provider Specialty | Specialty of the provider to search. |
| Provider Type | Type of provider to search. The value of "ALL PROVIDER TYPES" will be ignored. All other criteria will be used in this case. |
| Records | Number of records to display in the panel at one time. |
| Sort Results By | Field names on which the search results can be sorted. Values it contains:-

| State | State of the provider to search. |
| Zip | Zip code of the provider to search. |
